Pediatric General Medical Intake

Chief Complaint (CC):

Priyanka is reported to be unusually tired and has been in bed for two days, accompanied by persistent coughing and fever.

 

History of Present Illness (HPI):

Priyanka, typically energetic, has experienced increased fatigue over the past two days, leading to prolonged bed rest. She has had a persistent cough described as continuous and non-stop. Fever has been present for most of the last couple of days, with the highest recorded temperature of 39.5°C. The family has administered Children's Acetaminophen, which provides some relief for the fever but does not alleviate the cough. No information on exposures, sore throat, gastrointestinal symptoms, or other contextual factors was provided.

 

Review of Systems (ROS – pediatric):

- Fever: Present for most of the last two days, max 39.5°C

- Respiratory: Persistent, non-stop cough

- Behavior: Increased fatigue, decreased activity level (bedridden)

- No mention of sore throat, gastrointestinal symptoms, skin changes, musculoskeletal complaints, urinary symptoms, appetite, or hydration status.

 

Past Medical History (PMH): No past medical, surgical history, allergies, birth history, or family/social context provided.

 

Current Medications & Treatments: Children’s Acetaminophen administered for fever; partially effective for fever but cough persists.
